New Patient Registration

The doctors welcome new patients who live within our practice area. If you move out of the practice area you may need to register with another practice.

All new patients are invited to make an appointment to have a health check with our practice nurse.

Register With Us

To register with us, please check you live within our catchment area; you can do this using the map below.

Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration’ form.

When Registering Please Provide:

  • A copy of your ID
  • Proof of Address (This is to check that you live in the local catchment area)
  • We may also ask you for details of your previous GP surgery and basic health and medical information

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website. 

Go to the NHS entitlements migrant health page on the Government website.

If you are away from home, and staying within our catchment area, you can register as a temporary patient with St Agnes Surgery for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery.

This can be helpful if you’re living away from home but do not want to change your home GP surgery.

To register you’ll need to fill out a temporary services form (GMS3), available from Reception.

After three months, you will have to re-register as a temporary patient or permanently register with us.

Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP surgery registration form on their behalf. You will need to provide a form of ID to prove that you’re their parent or guardian.

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.

You can fill out a change your contact details form to let us know about a change in your address.
